The Health Savings Plan option includes a Health Savings Account with a Company contribution. – Dental Insurance Dental insurance through Delta Dental of IL offers an extensive dental provider network. Preventive visits are covered at 100%, which includes x-rays and two cleanings per year. Dental insurance includes orthodontia coverage for children and adults. – Vision Insurance Vision insurance through Superior Vision offers a large nationwide provider network. Yearly exams are covered at 100% and a yearly allowance is offered for frames or contacts. Referral bonuses, $750 for referring a full-time employee or $250 for referring a part-time employee, are payable after 90 days of employment. – PTO/Vacation Full-time employees receive annual Paid Time Off/Vacation.
